Сучасні підходи до аналізу стану кісткової тканини в прогнозуванні стабільності дентальних імплантатів

Мета. Проаналізувати сучасні дані щодо діагностичної та прогностичної значущості методів оцінки стабільності дентальних імплантатів, окремо розглянути можливості фрактального аналізу кісткової тканини та визначити невирішені питання.
Матеріали та методи. Проведено пошук у базах даних PubMed/MEDLINE, Scopus, Web of Science та в реєстрі ClinicalTrials.gov за період 2016–2026 рр. Пошук виконано за шістьма тематичними запитами англійською мовою. До огляду включено 28 джерел, що відповідали критеріям включення: клінічні дослідження за участю людей, систематичні огляди та метааналізи, експериментальні дослідження біомеханічних характеристик системи «кістка–імплантат», а також роботи з методології фрактального аналізу зображень щелеп.
Результати. Торк введення та резонансно-частотний аналіз відображають різні фізичні характеристики системи «кістка–імплантат»; дані щодо зв’язку між ними суперечливі. Прогностична значущість інтегральних показників щільності кісткової тканини обмежена: щільність пояснює 42,3 % дисперсії коефіцієнта стабільності імплантата, разом із шириною альвеолярного гребеня — 53,9 %. Комбінація структурних характеристик трабекулярної кістки за даними конусно-променевої комп’ютерної томографії пояснює до 71 % варіабельності стабільності. Фрактальна розмірність значуще корелює зі стабільністю, проте її обчислення не стандартизоване.
Висновки. Розробка стандартизованого протоколу фрактального аналізу кісткової тканини за даними конусно-променевої комп’ютерної томографії є передумовою впровадження методу в прогнозування стабільності імплантатів. Перспективним напрямом є створення прогностичних моделей, спрямованих на прогнозування динаміки стабільності, а не її значення в окремій точці спостереження.
